The Clinician will be responsible for working with patients and their families. This will include social assessments, individual / family / marital / counseling, group therapy, patient/family education, discharge planning and other modalities as applicable.

Job Duties/Responsibilities :

  • Completes psychosocial assessment as per policy on patients and reassesses as necessary for all patient populations and makes recommendations to the case manager and to the multidisciplinary team as evidenced by direct observation and documentation review with knowledge and skills necessary to provide care appropriate to the age of patients served and the most common diagnoses found in the behavioral health center.

  • Collaborates with the multidisciplinary team to develop and individualize discharge plan for each patient that is referred identifying patient psychiatric and social problems from assessment and enters them into the care plan with measurable interventions for age groups served.

  • Assesses all referrals of alleged abuse or neglect and makes referrals to appropriate protective agencies and advises the care team regarding the safety of the proposed discharge plan as evidenced by direct observation and documentation review.

  • Facilitates multi-disciplinary consultation and collaboration as needed with physicians, nurses, nutritionist, case manager, psychiatrist and pharmacist and other members of the treatment team as needed to provide comprehensive and optimal patient care and documents in weekly team meetings Will coordinate and mobilize community resources and patient's support system as part of the discharge planning process and/or transfers out of the facility.

  • Other duties as assigned.

    Qualifications

    Requirements:

  • Master’s degree in social work or master’s degree in counseling, education, or related field.

  • Minimum of 3 years work experience
