Make a Difference

The Ambulatory Collaborative Care Registered Nurse position is a Grant Funded position and will cover primary care on the east side. The RN provides enhanced nursing skills to the ambulatory primary care practice. The RN provides direct patient care, focuses on expanding primary care capacity, patient outcomes, satisfaction, and the reduction of cost through population health management. The primary focus will be caring for patients and families in the clinic setting, promoting their physical and mental health which includes, but is not limited to, pain management, wellness, and the prevention of illness. Implements and sustains patient-centered services for the aging and complex primary care population through chronic disease management. Uses health assessment skills to document assessment of personal and familial health within a social and environmental context (understanding the social determinants of health). May follow a panel of patients with engagement through nurse-led individual and group visits, health coaching and preventative health teaching. Actively participates and takes a leading role in health promotion and preventative care initiatives to promote positive health outcomes for our patients. Functions in the role of preceptor for RN student placements. The position will be located in Greenfield, Indiana.

Exceptional Skills and Qualifications

-Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.

-Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).

-Two years or more of nursing experience required.

-Excellent communication skills.

-Excellent team skills.

-Compassionate and caring.

-Accountable and self-directed.

-EPIC experience a plus.

-ASN required. BSN preferred.
