Req# 28967 Aultman Health Foundation, NORTH CANTON, OH AULTMAN N CANTON MEDICAL GROUP Aultman Health Foundation Full Time, Day Shift, M-F 8am-5pm *LPN's are eligible for a $5,000 Sign On Bonus with a 1-year commitment to Aultman. *MA's are eligible for a $2,400 Sign On Bonus with a 2-year commitment to Aultman JOB TITLE: Licensed Practical Nurse GENERAL STATEMENT OF DUTIES: Provides professional nursing care under supervision for clinic patients. ORGANIZATIONAL RELATIONSHIPS: Reports directly to the Practice Manager/Director of Practice Management. ESSENTIAL JOB FUNCTIONS: Duties performed in this position include but are not limited to the following: 1. Performs general nursing care to patients. Administers prescribed medications and treatments in accordance with nursing standards. 2. Assists with the preparation of equipment, and aids physician/nurse practitioner during treatment, examination, and testing of patients. 3. Observes, records, and reports patient's condition and reaction to drugs and treatments to physicians. Administers prescribed medications as directed. 4. Oversees appointment bookings and ensures preferences are given to patients in emergency situations. Maintains timely flow of patients. 5. Greets patients and prepares them for provider examination. Screens patients for appropriate information. Instruct patients in collection of samples and tests. Arranges for patient testing, referrals and admissions. Responds to and refers incoming telephone calls. Instructs patient and family regarding medication and treatment instructions. Formulates and updates patient care plans. Orders, prepares, and inspects patient's charts. Posts test and examination results. Responsible for preparation of exam rooms including re-stocking of treatment areas. Identifies clinical and office supplies and equipment needed and notifies lead and/or practice manager Attends required meetings and participates in committees as requested. 11. Participates in professional development activities and maintains professional affiliations. 12. Maintains patient confidentiality. 13. Performs related work as required per department. (See required duties specific to department.) 14. Comply with all statutory and regulatory policies and procedures 15. Teaches patient education 16. Delegates job duties. 17. Updates health maintenance 18. Double checks medications and immunizations. 19. Cross trained to check in/check out patients, make appointments, work front desk. 20. Prepares charts 1 week in advance gathering all outside consults/diagnostic testing/lab work ordered and performed since last visit. (Currently at 3 weeks out) Prints Enhanced Encounter Forms Identifies Medicare Wellness patients Optum forms as directed by Quality - LPN prints now 21. Completes patient intake, reviews and updates medical history and medications a. Verifies patient pharmacy 22. Gives injections and applies dressing as required a. Obtains and runs testing as ordered (urine dip, glucose, EKG, PT/INR, urine drug screen, blood work, urine pregnancy, rapid strep, rapid influenza) 23. Performs nurse visits - including education of physician's orders to patient and proper charting after visit 24. Assist provider with procedures (Pap and minor procedures) 25. Suture and Staple removal 26. Help process prior authorization of medications and testing 27. Maintain patient files, records, and other information a. Obtains information from paper charts or Allscripts (as needed) 29. Print Primetime and Optum forms 30. Monitors Clinical Pool and personal inbox 31.
